Your medical record is a confidential document. It is always the policy of this practice to maintain security of patient personal health information and to ensure that this information is only available to authorised members of staff.
We ask that you assist us in providing the best possible health care to you by making sure that the information we have is up to date.
If you wish to view your medical records, please discuss this with your doctor.
Please note: We cannot give out any information to anyone including other members of your family without your permission.
To comply with the standards of general practice we are required to go through an Accreditation process every three years.
Our next Accreditation is January 2022
As part of this re-accreditation process we may ask you to participate in a confidential patient survey.
These surveys are aimed at collecting your views on our practice so we can continually improve our services.
At this site visit another General Practitioner other than our own and a non-GP surveyor visit us.
Your medical case notes may be requested to be viewed as part of our accreditation.
Please Advise Reception if you do not wish your notes to be viewed as part of our accreditation process and we will flag your file.
